Billing and Insurance

Medical Insurance Plans

New patients should bring proof of insurance, as failure to do so may unfortunately delay or prevent you from seeing the doctor. Patients under an HMO health plan should also check their benefits prior to scheduling, as you may require a referral from your primary care provider prior to seeing a specialist physician.

We accept a wide variety of insurance plans, though there is some variability and not all doctors are on all plans. Most PPO and HMO carriers are accepted, but it is the patient's responsibility to provide our office with a doctor's referral if required by their insurance provider.

Fees and Billing

We will discuss all procedures, fees and financial arrangements prior to treatment at your request. Your insurance co-payment amount will be paid with proof of acceptable/current medical insurance coverage provided and on file. Full payment for services is to be made the day services are rendered, unless prior arrangement has been made. Please do not hesitate to ask questions regarding fees, charges and/or treatment.

Payment Options

The patient is financially responsible for their copay as well as any remaining balance following insurance payment. Acceptable forms of payment include Visa, MasterCard, Discover/Novus and American Express credit cards, and personal checks with proper identification.

ATX Medical Billing

An outside agency, ATX Medical Billing, handles all insurance filing and patient billing. However, you may make payments in the office at anytime or mail payments as directed by your billing statement.
